Acceptance of applicants who request homecare services is based on a reasonable expectation that the applicant’s medical, nursing, and psychosocial needs can be adequately met in the applicant’s place of residence. The following criteria must be met for care to be rendered by the agency.
- The applicants’ requested services are within the agency’s service area.
- An appropriate payer source is identified and services requested can be provided under a payer that the agency accepts, and payer has authorized and approved the care requested.
- The applicant’s requested care is medical in nature, and a provider (MD, DO, Physician Assistant, Clinical Nurse Specialist or a Nurse Practitioner, who is currently licensed in Indiana and is in good standing) agrees to provide orders for the care requested.
- The services requested are services the agency is able to provide (Skilled Nursing, Home Health Aide, Attendant Care, Homemaker/Companion).
- The agency has the available staffing for the requested services and has staff who possess the skills and competency to provide the required care needed to meet the patient’s needs.
- The applicant understands the agency does not employ family members to provide medical care needed.
- The applicant understands that the agency does not provide transportation for the applicant. Employees are not permitted to transport patients.
- An applicant who has services that would be considered “under arrangement” (more than one agency), is able to have needs met and care coordinated between both agencies. Advantage Home Health Care will not serve as the primary agency. Both agencies agree to sign a Coordination of Care Agreement acknowledging that they are responsible for providing the services listed.
- The applicant’s residence is maintained in a manner that is safe and is free from verbal, physical and psychological abuse for both the applicant and the employees of the agency.
- The applicant can be safely maintained in the home setting.
- The applicant is safe to be left alone in the home or has a primary caregiver in the agency’s absence.
